To complete last comment, "effectively uninsured" was a very poor (and apparently unsourced) way of trying to describe the problem. If you have a policy, you have a policy. If you don't, you don't. What the language was really trying to say is that a high copay defeats the purpose of insurance from the individual point of view, financially speaking. But if someone wants to add back, we can phrase more precisely.
